Flu Clinic October 13th 2016

Salado Elementary will be hosting the annual Flu Vaccine Clinic on Thursday October 13th.  The CDC has recommended that only the inactivated shot be given for this flu season and the mist will not be available.  Therefore we can only offer the inactivated shot for this year’s flu clinic.  The cost is $10.00 per student and they accept cash or check (please make out to Movax).  Please note that students who have not turned in forms and money by October 7th will not be able to participate.  The company needs to know in advance how many inactivated vaccines to have available that day.  FORMS AND MONEY DUE BY OCTOBER 7th, 2016.

 

Attached are the forms for the flu clinic.

 

Please turn in the following to the school nurse if you would like your child to participate in the flu clinic:

 

  • Immtrac form- If you do not wish to register with Immtrac, please fill out top portion and place an X over bottom half.
  • Inactivated (shot) form.
  • $10.00 cash or check made out to Movax; Please submit with forms
